ADULT DAY HEALTH CARE CENTERS
Driver
We are currently looking for a (PRN) Driver for the North Shore area. It is important the driver live near or in the surrounding Covington area as the person will be required to drive to & from Slidell, Covington & Hammond Mon-Fri. This position is responsible for transporting program clients to and from assigned destination, assisting clients as needed entering and exiting van. Able to complete assigned deliveries and pick-ups as requested. Reports vehicle maintenance issues, maintains program & vehicle logs as required, maintains and operates vehicle in a safe manner. A successful candidate will have previous experience in van and bus driving; working with the elderly/disabled adults is helpful. All successful candidates must have a valid Louisiana driver’s license and a minimum of a high school diploma or a GED.

FJC Investigator
Works directly with on-site legal partners, under the supervision of the NOPD DV Unit, to assist with follow-up-gather pertinent information and evidence, prepare concise written reports, operate investigative equipment, establish and maintain good working relationship with municipal court city attorney and social service agencies. Previous law enforcement experience and post certification preferred. Must be knowledgeable of victim’s rights and domestic violence laws and have good oral and written communication skills.

Parish Social Ministry Coordinator
The Parish Social Ministry Coordinator is responsible for developing parish social ministries, coordinating Operation Rice Bowl and coordinating the Catholic Campaign for Human Development parish educational and organizing programs. This would include coordinating all aspects of parish social ministry to include outreach, training, resourcing and follow-ups to meet the need of parish lay leadership. Social action skills with an ability to do social analysis with an understanding of how change actually happens Ability to recruit, motivate, lead other staff and volunteers. Knowledge of church social teaching and be able to articulate it clearly. Awareness of the need for a rapport with the poor and sensitivity to the variety of cultures. Undergraduate or graduate degree preferably in Theology desired.

Administrator Assistant Payables
Experienced professional to track payables and compile site inventory. Must be proficient with MS Excel. Strong organizational; skills and interpersonal skills required. BA preferred; 5 years experience with payables and spread sheets. 20 hours weekly.
Hospitality Coordinator
Individual needed for volunteer support, with high organizational skills and attention to detail. Good communication skills a must. REQUIREMENTS: Able to lift 30 lbs, associates degree or 5 years experience in non-profit organization. Bachelor’s degree preferred.
Youth build Construction Trainer
Skilled trainer needed to train individuals in the skills for the rehabilitation of hurricane damaged houses for a special job training program for youth ages 16-24. Duties include:
- Teach construction skills including framing, interior, carpentry and the use of equipment through hands on approach.
- Provide instruction in the use and handling of tools, equipment and construction site management.
- Work in partnership with team members to assist with personal and vocational counseling of program enrollees.
Qualifications include a high school diploma, and 5-7 years journeyman level building trade experience. Knowledge of basic building codes and all aspects of home construction necessary. Strong interpersonal and mentoring skills required.
PACE (Program for All-Inclusive Care for the Elderly)
Certified Nursing Assistant
High school Diploma/GED, Current State Certified Nursing Assistant License, one year basic experience with the frail or elderly, good interpersonal skills and the ability to work with geriatric patients and handle unusual interpersonal situations. Must be able to maneuver and operate general household equipment. Work under the general supervision of the nursing staff but reporting to the Center Director, is responsible for the provision of direct care in the PACE GNO ADHA, including but not limited to personal care, non skilled treatments, assistance with toileting, feeding, recreational activities and escorting participants to and from physician appointments or other services outside of the PACE GNO ADHC.
Director’s Assistant
Responsible for the overall maintenance of all PACE records in accordance with the state and federal law. Coordinates all data entry in compliance with state and federal regulations; schedules participant’s appointments, coordinates the authorization process for all outside appointments. The Director’s Assistant is responsible for providing administrative support including organization, implementation and completion of multiple projects for the Center Director. A college degree, proven proficiency in typing, a high degree of proficiency with MS Office products including Word, Excel, Access, Power Point and Outlook, experience in technical writing is required. Must be able to maintain the highest level of confidentiality. Handle sensitive material concerning the organization, its program and staff. Writing samples required.
Registered Nurse
Provide direct and indirect nursing care to assigned patients. Works under the direct and indirect supervision of the Center Director or designee but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which he/she is responsible. This position supervises both licensed and non-licensed personnel. A qualified applicant will be a graduate of an accredited school of nursing and hold a current RN license. He/she must have practiced nursing in the last three years. The position requires eighteen months RN experience with at least twelve of those months in geriatric practice area preferred. The individual must present credentials which demonstrate an ability to meet performance criteria for the position and posses knowledge and skills necessary to treat geriatrics patients.
Dietician
Responsible for providing nutrition care and education to PACE participants. Evaluate the participant’s nutritional status, develop and implement nutritional care plans, assesses nutrients requirements, recommend nutritional prescriptions, evaluate participant tolerance and effectiveness of nutritional care and communicate nutritional data and information to the physicians and other health professionals. Bachelor’s Degree from an ADA accredited institution with a major in nutrition. Must be ADA registered. CDE status preferred but not required, should be CDE eligible within 24 months. Masters Degree plus approved work experience preferred. One year experience working with the frail and elderly.Able to teach, think analytically and relate to geriatric patients. Knowledge of clinical dietetics mandatory.
Staff Accountant
Needed to maintain a variety of general ledge accounts which includes preparation of reconciliations and account analysis. Assist with monthly financial closing and preparation of financial statements. Review and approve provider’s invoices for repayment. Review and reconcile monthly payments from CMS for accuracy and completeness. Responsible for analyzing monthly variances and assisting the Director of Finance with other tasks as assigned. Bachelor’s degree in accounting or Finance.
Nurse Practitioner
Needed to assess and manage the healthcare of chronically ill geriatric participants in the PACE program. Bachelor’s Degree in Nursing and graduate of certified or accredited Nurse Practitioner, or a Master’s Degree in Clinical Nursing and five years nursing experience, some of which must be with a geriatric population. Must have a valid RN license in Louisiana.
Home Care Technician
Performs patient care activities primarily in the community with some activities in the Day Health Canter. Provides personal care to participants in their homes (bathing, dressing, feeding, toileting, transfer, lifting, incontinence, and catheter care). Provides basic housekeeping to participants (laundry, meal preparation and clean-up, general cleaning). Medication monitoring and cueing. Responsible for reporting any changes in patient’s medical or physical status to RN supervisor. Perform procedures/treatments under direct supervision of RN. Independently performs superficial wound care and dressing changes as ordered. Documents services provided and observations on appropriate forms. Must be able to move about freely, lift and turn patient. Requires manual dexterity and visual acuity to see and treat patients; able to hear and orally communicate. Must be able to maneuver and operate general household equipment. High school diploma/GED with at least three months of experience as a nursing technician/nursing aide, corpsman, or EMT. Must posses a valid driver’s license and reliable transportation. Good driving skills, good interpersonal skills and the ability to work with geriatric patients and handle unusual interpersonal skills.
Home Care Coordinator
Under the direct supervision of the Center Director responsible for the development and implementation of home care services fir the PACE participants, including the coordination of all durable medical equipment. Performs home visits, oversees home health contract provides and participates in the Interdisciplinary Team. Registered with 3-5 years of experience in home health care, hospice, geriatrics, case management, or related field. Must be licensed as an RN in Louisiana and possess a valid driver’s license with reliable transportation. Position requires frequent travel in and around New Orleans area. Able to function well in a team environment. Must have good organizational;. Leadership, interpersonal relationship, multi-task coordination, oral and written communication skills.
Transportation Coordinator
Under the general direction of the Center Director, plans, organizes and implements the transportation services of the PACE Center. Enters into private homes of PACE enrollees, frequently in unsafe neighborhoods. Plans and coordinates all scheduling activities related to transportation for PACE participants. Supervises the daily activities of the Drivers/Aides. Develops monthly transportation schedules and updates schedule as needed. Responsible for coordination of all outside service transportation service providers. Reviews and verifies all invoices for services provided by outside contractors. Plans, implements and supervises all vehicle maintenance. Assures that all necessary program records are maintained according to the policy. Participates as a member of the multidisciplinary team. Completes all other job duties related to the care and well being of PACE participants. Must be able to see, hear and orally communicate effectively. Must have good manual dexterity and be able to lift and transfer patients. Visual acuity required assessing condition of participants. High school diploma or GED and at least to two years of supervisory experience. Will supervise three to six drivers/aides. Must have a valid Louisiana chauffeur’ license with a good driving record and possess the skills necessary to provide care for geriatric participants. Should be able to get along with others in a team situation. Must be able to operate specially equipped vans and have good mathematical and organizational skills.
PADUA COMMUNITY HOMES
Residential Counselor
Provide active treatment to individuals with developmental disabilities in a community home. Participate in planning for individual active treatment including community integration activities. High school Diploma and previous experience working with the developmentally disabled persons preferred. Good writing and communication skills, a must.
Service Coordinator
Coordinate services for five ICF/DD community homes experience working with developmental disabilities and medically frail persons a plus. Excellent written and oral communications skills required. Bachelor degree in Human Services or a related field.
VOYAGE HOUSE
Therapist
Full-time individual needed to facilitate groups, provide individual therapy to homeless women ages 39 and older. LCSW or LPC preferred MSW with 5 years of experience may be considered.
